The Division of Prevention and Behavioral Health Services Survey

The Division of Prevention and Behavioral Health Services (DPBHS) is seeking to better understand the needs of the community in regard to mental health services for youth who reside in the Smyrna School District.

The Division also hopes to gain a better understanding of people’s knowledge of the services currently available. Please consider completing the survey linked below to provide your feedback. The survey can be completed by students, parents, staff, and members of the community. Your feedback is valuable to determining how the Division can better meet the needs of youth in our area.
